| Before you arrive

1. Computerized National Identity Card (CNIC) if you are above 18 year
2. Mobile Phone Number

If you are minor (Less then 18 years), please bring at least mobile phone number of your or your either parents.

The serving employee of P.R. shall furnish the following documents:

1. Valid Employment Card; and
2. 92 Form (where applicable); and Copy of CNIC                 

The following documents shall be required for the provision of healthcare services to the dependent family members of railway employee:

1. Dependents’ Valid Entitlement Card; and
2. Copy of B-Form*
3. Birth Certificate for infants or who has not been registered as dependent
4. Valid Employment Card of principal railway employee;

*’Form-B’ may be used as an option to identify the dependency in case of unavailability of ‘dependent entitlement card’ but can’t be used as a substitute of ‘dependent entitled card’.

The pensioner shall provide the following documents:

1. Valid Pensioner Card
2. Copy of pension book (first three pages)
3. Copy of CNIC                         

The widow/widower of deceased employee shall furnish the following documents:

1. Valid Widow/Widower Card
2. Copy of pension book (first three pages

All entitlement cards such as ‘employment card’, ‘dependent cards’, Pensioner card’, and ‘widow /widower card’ must be signed and stamped by the Divisional Personnel Officer (DPO) or an authorized officer of Pakistan Railway Rawalpindi Division
